Not in a way the integrity-screened trials could measure: in the review written for the 2023 international PCOS guideline, BMI did not differ from placebo, and BMI, weight and waist circumference did not differ from metformin.1 A separate 2023 pooling that reported a small fall in BMI counted a 2007 trial of 92 women that was retracted in 2023.2 No trial has tested inositol for weight in anyone without polyendocrine metabolic ovarian syndrome (PMOS), the NHS's name since 2026 for polycystic ovary syndrome, PCOS below. One spoke of the Wellgard inositol guide.

Evidence at a glance

  • Trials: 30 randomised trials (19 pooled), 2,230 women with PCOS, in the integrity-screened 2024 review for the 2023 guideline.1
  • Largest single trial: 132 women in India, 4 g of myo-inositol a day for 12 weeks, in the nine-trial 2026 pooling.3
  • Strongest measured outcome: BMI against the folic acid comparator, three trials, 112 women, mean difference 0.14 units, not significant, very low certainty.1
  • Not tested: inositol for weight in anyone without PCOS, and belly fat measured by scan.

Does inositol help with weight loss? What the trials measured

The 2023 international PCOS guideline commissioned its own review, published in 2024, which screened every trial for research integrity before pooling: 43 trials were assessed, 30 of 2,230 women were included, and 13 were set aside over integrity concerns.1 Against folic acid alone, the comparator those placebo-style trials used, BMI did not differ in three trials of 112 women.1 Against metformin, BMI, body weight and waist circumference did not differ, and waist-to-hip ratio improved more on metformin.1 The authors call the evidence "limited and inconclusive" and "very uncertain for body mass index".1

The guideline turned those rows into two recommendations: inositol "could be considered", "noting limited harm, potential for improvement in metabolic measures, yet with limited clinical benefits including in ovulation, hirsutism or weight"; and "metformin should be considered over inositol for hirsutism and central adiposity, noting that metformin has more gastrointestinal side effects than inositol".4 Inositol is a food supplement with no authorised health claim in Great Britain. What the same trials did record was movement in blood tests (fasting insulin, HOMA-IR, testosterone), set out in the Wellgard inositol guide.

Weight, BMI and waist outcomes for inositol in women with PCOS, from the integrity-screened 2024 review written for the 2023 international guideline.
Outcome Comparison Trials and women Difference Certainty
BMI Myo-inositol with folic acid against folic acid alone 3 trials, 112 0.14 units, not significant Very low
BMI Myo-inositol against metformin 6 trials, 420 0.03 units, not significant Very low
Body weight Myo-inositol against metformin 3 trials, 174 0.58 kg, not significant Low
Waist circumference Myo-inositol against metformin 3 trials, 220 2.5 cm, not significant Moderate
Waist-to-hip ratio Myo-inositol against metformin 2 trials, 160 0.04, better on metformin Low
BMI D-chiro-inositol against placebo 2 trials, 64 0.67 units higher on D-chiro-inositol Low

Why does the headline figure count a retracted trial?

The number most pages quote comes from a different 2023 pooling, 26 trials and 1,691 women, which reported BMI 0.45 units lower on inositol than on placebo, from eight trials of 419 women.2 That pooling applied no integrity screen, and its BMI pool counted a 2007 trial of 92 women that was retracted in 2023 as a duplicate of an earlier trial, and is not counted here. The guideline review's authors say the 2023 pooling "included some studies that we had excluded from analyses due to uncertainty about research integrity", and that it pooled before-and-after change scores, which "tend to produce larger effect sizes".1 Eleven clinical inositol papers now carry a retraction and seven an expression of concern; the Wellgard inositol guide names them.

Does inositol reduce belly fat?

No trial has measured belly fat as such. The trials in the guideline review recorded BMI (29 trials), waist-to-hip ratio (16), weight (12) and waist circumference (8); none measured visceral fat by scan.1 The two waist measures are the nearest thing: waist circumference did not differ from metformin, and waist-to-hip ratio improved more on metformin, which is why the guideline prefers metformin for "central adiposity", its term for fat carried around the middle.1, 4 Against placebo there is no pooled waist comparison. So the belly-fat question is answered by a count of zero trials, not by a verdict.

Who responded in the trials, lean or overweight women?

A 2026 pooling asked that question of the testosterone results: nine randomised trials, 440 women with PCOS (225 on an inositol, 215 on placebo or folic acid), 6 to 24 weeks, none of the nine on the retraction or expression-of-concern lists.3 Total testosterone fell across the pool (a standardised difference of 1.30), and the largest, most consistent fall was in normal-weight women with a BMI under 25, in two trials of 62 women; overweight and obese women, and women with insulin resistance considered apart from BMI, showed no significant improvement.3 The authors call the subgroup result exploratory. It concerns blood tests, not weight. One trial in that set, 26 lean women on 1.2 g of myo-inositol a day for 12 weeks, recorded body weight 1.83 kg lower against a 0.25 kg rise on placebo; the guideline review rated it high risk of bias and very low certainty, and did not pool it.1

Does inositol help with weight loss without PCOS?

No trial has tested it. Every figure on this page comes from women with PCOS, a condition in which the NHS lists "weight gain or difficulty losing weight" among the main symptoms.5 The best-known adult trials outside PCOS are the mood trials: a 2014 pooling of seven depression trials (242 people) and four anxiety trials (70 people) measured symptoms, found no significant effect on either, and reported no weight outcome.6

How does inositol compare with the medicines people ask about?

The NHS PMOS page lists "weight loss medicines if you're overweight" and "metformin, a type 2 diabetes medicine to help tiredness and weight problems" among the treatments a doctor may offer; neither is a supplement.5 A 2026 network analysis of 16 randomised trials in women with PCOS compared the GLP-1 receptor agonist medicines, metformin and myo-inositol: a GLP-1 medicine with metformin reduced weight by 5.58 kg and a GLP-1 medicine alone by 5.22 kg, with a waist 4.7 cm smaller, and none of the treatments changed HOMA-IR significantly.7 Added to metformin, in six trials of 388 women over three to six months, inositol made regular cycles more likely and lowered hirsutism scores, and BMI did not differ.8 Nobody starts, stops or swaps a prescribed medicine on the strength of a supplement trial; food for PCOS covers eating on those medicines, and inositol and metformin, what the trials compared has the head-to-head record. Berberine's weight record is on does berberine work for weight loss; no trial has tested the two together.

What does the guideline say to do about weight with PCOS?

Lifestyle change sits at the centre of the 2023 guideline, with medicines chosen for the problem at hand and inositol an option that "could be considered", with limited clinical benefit for weight.4 A NICE draft guideline for the UK (consultation closed 11 August 2026; publication expected 27 January 2027) goes further than "could be considered": it would have clinicians advise that "there is uncertainty about the clinical benefit of inositol for metabolic health problems"; until it is published it is a draft, not guidance.9 The NHS page says the same in plainer words: eat a healthy, balanced diet, exercise regularly, try to lose weight if you're overweight, and NHS Better Health has free tools and support.5 Weight that will not shift despite those changes is a GP conversation, where metformin and the weight-loss medicines are options. What the trials recorded as side effects is on inositol side effects.

What we don't know

  • No pooled weight comparison for inositol runs past six months: the guideline review's head-to-head trials ran three to six months and the 2026 pooling's 6 to 24 weeks.1, 3
  • No trial has measured visceral fat by scan; waist circumference and waist-to-hip ratio are the nearest measures recorded.1
  • No randomised trial has tested inositol for weight loss in people without PCOS.
  • Whether a responder can be identified in advance is untested: the lean-phenotype finding comes from two trials of 62 women and concerns testosterone, not weight.3
  • No trial has tested inositol added to a GLP-1 medicine.

Frequently asked questions

How long does inositol take to work for weight loss?

The trials give no time to quote, because they found no difference to time: BMI did not differ from the comparator, and BMI, weight and waist did not differ from metformin, over the three to six months they ran.1

Is inositol good for PCOS weight loss?

As the trials measured it: BMI did not differ from the folic acid comparator or from metformin, weight did not differ from metformin, and the guideline prefers metformin for central adiposity.1, 4 The 0.45-unit BMI fall quoted elsewhere counts a 2007 trial of 92 women retracted in 2023, not counted here.2

Does inositol make you gain weight?

Not in the myo-inositol trials, where BMI and weight did not differ from the comparators.1 D-chiro-inositol on its own is the one exception: in two placebo-controlled trials of 64 women lasting six to eight weeks, BMI ended 0.67 units higher on D-chiro-inositol, low certainty.1, 3 The guideline recommends no type, dose or combination of inositol.4

Can you take inositol with weight-loss injections?

No trial has tested the pair; the 2026 network analysis compared the GLP-1 medicines, metformin and myo-inositol as separate treatments.7 Anyone prescribed a weight-loss medicine asks the prescriber before adding a supplement.
