How Many Calories Should You Eat on Mounjaro?

Most people on Mounjaro should not be counting calories, because the medicine does the restricting for them; the number that matters is the floor, not the ceiling. In controlled studies, GLP-1 medicines reduce what people eat by about a quarter to a third with no instruction to eat less, and the big tirzepatide trials asked participants for a deficit of around 500 kcal a day below what they burned, with a diet built on protein and fibre.1, 2 That is a moderate deficit, not a crash diet. The problem in practice is the opposite of the one people expect: appetite falls so far that intakes of 800 to 1,000 kcal a day become normal without anyone noticing, and that is where tiredness, headaches, hair shedding, muscle loss, cold hands and stalled progress come from. For most women the floor is about 1,200 kcal a day and for most men about 1,500, and below it you lose more muscle, not more fat.

Why calorie counting usually backfires on Mounjaro

Dieting without medicine means fighting appetite with willpower, and counting is one way to do it. Tirzepatide, the medicine in Mounjaro, removes most of the fight: fullness comes early and lasts, hunger between meals fades, and food noise goes quiet.1 Counting on top of that tends to push people lower than they should be, because the number on the app feels like a score and lower feels like winning. It is not. So how many calories should you eat?

Enough that you are losing steadily, keeping muscle, and feeling well. In numbers:

  • A floor, not a target: about 1,200 kcal a day for women and 1,500 for men. These are the levels below which it is difficult to get enough protein, fibre and micronutrients from food, which is why NHS weight-management programmes do not go lower without medical supervision.
  • Most people land naturally at 1,300 to 1,800 kcal once the medicine has settled, depending on size and activity. If you are tall, heavy, very active or breastfeeding, the right level is higher.
  • The trial pattern was roughly 500 kcal below the amount that would maintain your weight, which for most adults means 1,400 to 2,000 kcal a day.2 That produced the trial results. Nobody needed 800.

If you want one practical check, it is this: three meals and a snack, each with a protein source, every day, with the portion sizes set by comfort. Hit that and you will be above the floor without an app.

Signs you are eating too little

Under-eating on Mounjaro does not feel like hunger, which is what makes it easy to miss. It feels like the medicine's side effects: tiredness that does not lift, headaches, feeling cold when others are not, dizziness on standing, constipation, poor sleep and waking at four in the morning, hair shedding two to four months in, irritability, and a stall on the scales despite "doing everything right". The tell is that eating more fixes it within days. How to eat enough when you have no appetite

Schedule it. Hunger will not remind you, so the clock has to. Something with protein within an hour of waking, then every three to four hours.

Protein first, always. Appetite suppression hits protein foods first because they are the filling ones. Make them the start of every meal: eggs, yoghurt, cottage cheese, fish, chicken, tofu, lentils, a shake if you cannot face food. How much protein you need has the figures.

Choose dense, not bulky. When you can only manage a small volume, make it count: full-fat Greek yoghurt rather than low-fat, nut butter on toast, olive oil on vegetables, avocado, cheese, whole milk in porridge, a handful of nuts. Soups and smoothies carry protein and calories when solid food is unappealing.

Do not drink your room away. Fluid between meals, not with them, so water does not fill the space food needs. Aim for 1.5 to 2 litres across the day.

Treat a sub-1,000 day as a problem. One is fine. A pattern of them is the time to raise it with your prescriber, because the usual answer is to hold at the current dose until eating recovers, and that is their call, not yours.

What about tracking?

If you like tracking, track protein and fibre rather than calories: 1.2 to 1.6 g of protein per kilogram and 30 g of fibre a day are targets to reach, and reaching them keeps you above the floor automatically. If you want to track calories for a week to see where you are landing, do it as a check, not a diet, and stop once you know. People with a history of disordered eating should not track at all and should tell their prescriber that appetite suppression is a risk for them.

What we don't know

  • The ideal intake on tirzepatide. The trials prescribed a deficit; nobody has compared intakes against outcomes.
  • How far resting metabolism falls with very low intake on these medicines, and how much protein and training offset it.3, 4
  • Whether under-eating early predicts regain later. Plausible from the weight-maintenance research; unstudied here.
