What Should You Not Take With Zinc? Iron, Medicines, Food
Nothing has to be avoided outright with zinc, but four things need a gap: iron supplements, ciprofloxacin and the other quinolone antibiotics, the tetracycline antibiotics (doxycycline, lymecycline, oxytetracycline and tetracycline) and penicillamine. Vitamin D, vitamin C and magnesium can go alongside it, so "what should you never take zinc with?" has a timing answer, not a ban. UK figures first: about 9.5 mg a day for men (aged 19 to 64 years) and 7 mg a day for women, and the NHS line "Do not take more than 25mg of zinc supplements a day unless advised to by a doctor".1 This is the interactions chapter of the Wellgard zinc guide, within the vitamins and minerals guide.
- Leave 2 hours: quinolone antibiotics; a standalone iron supplement
- Leave 2 to 3 hours: doxycycline, lymecycline, oxytetracycline, minocycline, tetracycline
- Ask first: penicillamine, levothyroxine, alendronate, risedronate, diuretics
- Fine together: vitamin D, vitamin C, magnesium, folic acid, B12, omega-3
- With a meal if it makes you feel sick; add up the zinc across products
What should you not take at the same time as zinc?
Zinc is a positively charged mineral. In the gut it binds certain antibiotic and bone-medicine molecules so the medicine is poorly absorbed, and it shares an uptake route with non-haem iron.2, 3 Both are timing problems: once a medicine has been absorbed, zinc taken later does nothing to it.
Which medicines interact with zinc, and how far apart should you take them?
The table quotes the NHS medicines pages and NICE's BNF interactions list. Generic names only; your own leaflet comes first.
| Medicine | What the UK source says | Gap from zinc |
|---|---|---|
| Ciprofloxacin, levofloxacin, moxifloxacin, ofloxacin | NHS: iron, "calcium and zinc supplements can affect ciprofloxacin. Leave 2 hours in between taking supplements and your dose of ciprofloxacin tablets or liquid."4 BNF: 2 hours for each2 | 2 hours |
| Doxycycline | NHS: "medicines or supplements that contain iron or zinc" "may not mix well with doxycycline"5; BNF: "separate administration by 2 to 3 hours"2 | 2 to 3 hours |
| Lymecycline | NHS: tell your doctor about "any supplements that contain aluminium, bismuth, calcium, magnesium or zinc"6; BNF: 2 to 3 hours2 | 2 to 3 hours |
| Tetracycline, oxytetracycline, minocycline | BNF: 2 to 3 hours.2 In 7 volunteers, 45 mg of zinc cut tetracycline absorption by about 30%; doxycycline "was not influenced significantly"7 | 2 to 3 hours |
| Penicillamine | BNF: "Zinc is predicted to decrease the absorption of Penicillamine. Manufacturer makes no recommendation."8 | Ask the prescriber |
| Levothyroxine | NHS names "antacids and other indigestion treatments" and "calcium and iron supplements" as things that "may not mix well"; zinc is not named9 | Ask your pharmacist |
| Alendronate, risedronate | BNF: zinc decreases their absorption; alendronate "at least 30 minutes before" zinc, risedronate "at least 2 hours"2 | As the BNF states |
| Thiazide diuretics, some ACE inhibitors and ARBs | A review of 8 small studies found higher urinary zinc losses with captopril, enalapril, losartan and hydrochlorothiazide10 | No gap; never change the medicine; tell your GP |
For the acne antibiotics lymecycline and doxycycline, the simplest arrangement is the antibiotic in the morning and any zinc-containing supplement with the evening meal. Penicillamine is different: the UK Expert Group notes that zinc sulphate "is used to treat Wilson's disease", so anyone with that diagnosis takes zinc only as their specialist directs.12
Can you take zinc and iron together?
In a multivitamin, yes; as two separate supplements, space them. Non-haem iron and zinc share an absorptive pathway, and the inhibition becomes measurable at an iron-to-zinc ratio of 2:1 or more "when the total amount of ionic species is greater than 25 mg"; pregnant women on high-iron prenatal supplements showed a smaller circulating zinc pool.3 Iron in a meal does not have this effect; iron in the same supplement does.11 So a multivitamin carrying 14 mg of iron with 10 mg of zinc sits at 1.4:1 and 24 mg, under both thresholds, while a standalone iron supplement plus a separate zinc product crosses them: different meals. See foods high in iron and iron supplement side effects.
Can you take zinc and calcium together?
Yes, with one caveat about tablets. The UK Expert Group on Vitamins and Minerals noted that "high levels of calcium in the diet can decrease zinc absorption", and the BNF lists the calcium salts as decreasing zinc absorption, with "Manufacturer makes no recommendation" on timing.2, 12 The BNF sets no timing, so the two-hour gap on do you need a calcium supplement is a precaution, not a trial result.
Can you take zinc and magnesium together?
Yes. Magnesium does not appear in the BNF's 28-entry zinc interaction list; the only official line is the EVM's "Excess zinc may also decrease magnesium and calcium uptake", which is about excess.2, 12 What should you not take with magnesium is this page's mirror; can you take zinc and magnesium together covers the pairing.
Can you take vitamin D and zinc together?
Yes. No interaction between vitamin D and zinc is documented in the NHS pages or the studies read here. Keep each within its UK line: zinc at or under 25 mg from supplements; vitamin D at 10 micrograms a day for adults, and "Do not take more than 100 micrograms (4,000 IU) of vitamin D a day as it could be harmful".13 Vitamin C: see vitamin C and zinc together; can you take too much vitamin D covers the ceiling.
Does copper matter?
Copper is why the 25 mg line exists. The NHS: "Taking high doses of zinc reduces the amount of copper the body can absorb. This can lead to anaemia and weakening of the bones."1 The EVM: "Zinc affects iron and copper uptake at supplemental doses of 50 mg/day and above. However, this is not apparent in all studies".12 No regulatory document sets a zinc-to-copper ratio; copper's NHS figures are 1.2 mg a day and "Having 10mg or less a day of copper is unlikely to cause any harm".1 Zinc and copper and can you take too much zinc take this further.
Which foods block zinc absorption?
Phytate, the phosphorus store in wholegrains, pulses, nuts and seeds. Across 30 phytate studies, fractional zinc absorption fell by 0.14, to 45% of control, when a meal's phytate-to-zinc molar ratio was above 15.14 The UK diet sits under that: the median adult ratio in the National Diet and Nutrition Survey was 9.7.15 Zinc from meat, fish, dairy and eggs carries no phytate penalty.
Should you take zinc on an empty stomach?
Not if it makes you feel sick. The absorption case is brief: zinc taken with water between meals "initially is absorbed more efficiently than food zinc, but absorption efficiency declines within 24 hours".16 The EVM recorded that stomach symptoms were "particularly apparent when the supplements were taken with little or no food", and in a 150 mg trial 84% of women reported symptoms such as nausea and cramps, five withdrawing for gastric irritation.12, 17 Dose and timing matter more than the salt, so: a modest dose, with a meal, at the same time each day. When is the best time to take zinc covers morning against evening.
Who should ask a pharmacist or GP first?
Anyone on a prescription in the table. Anyone with Wilson's disease, previous stomach or bowel surgery, or a malabsorption condition. Heavy drinkers, because alcoholism is a recognised cause of zinc deficiency and the liver is a GP matter.18 People with diabetes or haemochromatosis, named by the EVM as groups with "insufficient data" on the intake at which harm could occur.12 Pregnant and breastfeeding women, who stay within 25 mg and ask a GP or midwife first. And everyone on more than one product: a multivitamin plus a vitamin C and zinc capsule can together pass 25 mg, so add up every label (how much zinc a day).1
Frequently asked questions
What should you not mix zinc with?
Iron supplements, the quinolone and tetracycline antibiotics, penicillamine, and alendronate or risedronate: all spaced, none banned.2, 4
Can I take five different vitamins at once with zinc?
Yes, within each nutrient's UK line, with two checks: add up the zinc across the products, and take a standalone iron supplement at a different meal.1, 3
Can I take zinc with folic acid, B12 or omega-3?
Yes. No interaction with folic acid, vitamin B12 or omega-3 is documented in the NHS pages or the studies read here.
What helps zinc absorb better?
Protein in the meal, and phytase, germination and fermentation, which "markedly" improve absorption: a point for sourdough and soaked pulses rather than raw bran.11 Not the same mouthful as an iron tablet.3
