It depends on the fibre. Psyllium husk, a gel-forming fibre, cut leakage of loose stools in three small trials and firmed stools in one; a large, sudden dose of fast-fermenting fibre, or the sugar alcohol sorbitol, can loosen them. Diarrhoea with blood, signs of dehydration or lasting more than 7 days is a call to NHS 111, not a fibre question.1, 2, 3, 4 This page is part of the Wellgard fibre guide.
Evidence at a glance
- Trials: three randomised trials of psyllium for loose stools, 162 adults in all.
- Largest single trial: 80 adults with faecal incontinence, four weeks on each treatment.
- Strongest measured outcome: incontinent stools on fibre at less than half the placebo rate over 31 days.
- Not tested in these trials: diarrhoea from a stomach bug or food poisoning.

Psyllium is never taken dry: the NHS says at least 150 ml of cold water per sachet, drunk straight away, and not just before bed.5 Chest pain, vomiting, or difficulty swallowing or breathing after a dose needs urgent help; is psyllium husk safe has the rest.
When is diarrhoea not a fibre question?
Most diarrhoea is a stomach bug or food poisoning and usually stops within 5 to 7 days, the NHS says. "The most important thing is to have lots of fluids to avoid dehydration."4 Call 111 if you have bloody diarrhoea or bleeding from the bottom, diarrhoea for more than 7 days, keep being sick and cannot keep fluid down, or still have signs of dehydration after oral rehydration sachets; vomit that looks like ground coffee or a sudden, severe tummy ache means 999.4 A pharmacist can suggest oral rehydration powder, or a medicine to stop diarrhoea for a few hours such as loperamide, which is not for children under 12.4 Weight-loss injections have their own page on constipation and diarrhoea.
Which fibres firm loose stools, and which loosen them?
A gel-forming fibre such as psyllium holds water and is barely fermented, so, as a 2017 review describes, it normalises stool form in constipation and diarrhoea alike.6 Poorly absorbed fermentable carbohydrates can do the opposite, drawing water into the stool, as the pilot's authors explain.3
| Fibre or food | What the evidence found | Evidence |
|---|---|---|
| Psyllium (ispaghula) | Fewer leakage episodes in 3 trials; firmer stools in 1 | 3 randomised trials, 39, 80 and 43 adults with faecal incontinence1, 2, 3 |
| Gum arabic (acacia) | Fewer incontinent stools and firmer stools, as with psyllium | 1 randomised trial, 39 adults1 |
| Native inulin | Mild wind and bloating; up to 10 g a day well tolerated | 1 crossover trial, 26 healthy adults, supplier-funded7 |
| Oligofructose (short-chain inulin) | 10 g in one go substantially raised gut symptoms; 5 g well tolerated | The same trial7 |
| Inulin-type fructans with IBS | More wind than placebo | Subgroup of a pooled analysis of 11 trials, 729 people8 |

Does psyllium husk help diarrhoea? What three trials found
For adults with loose stools and faecal incontinence, it cut leakage in three small randomised trials and firmed stools in one. In 39 adults who leaked loose or liquid stools and took psyllium, gum arabic or placebo for 31 days, on either fibre the share of incontinent stools was less than half the placebo group's, and the placebo group had the most loose or liquid stools.1
In 80 adults, a double-blind crossover trial gave four weeks of psyllium powder and four of loperamide, an antidiarrhoeal medicine. Neither beat the other on episodes, severity or quality of life, both reduced episodes, and constipation affected 10% on psyllium against 29% on loperamide.2 In a pilot of 43 adults, 50% on 6 g a day of psyllium more than halved their episodes over four weeks against 38.9% on a dietitian-led low-FODMAP diet, a difference that was not statistically significant; only the diet group improved its severity score.3
These were people with bowel leakage, not a stomach bug; the NHS says anyone with leakage should see a GP, "as there are treatments that can help".12
Can too much fibre cause diarrhoea?
Yes, mainly a large, sudden dose of fast-fermenting fibre, and more so with IBS, though the trials measured wind and bloating more often than loose stools. In a supplier-funded crossover trial of 26 healthy adults, native inulin was well tolerated at up to 10 g a day, while 10 g of the shorter-chain oligofructose in one go substantially raised gut symptoms.7 In a pooled analysis of 11 trials of 729 people with IBS and similar bowel disorders, inulin-type fructans made wind worse.8
An IBS gut is more sensitive, not gassier: on MRI, 40 g of inulin made as much colonic gas in 29 people with IBS as in 29 without, but 13 of the IBS group reached their symptom threshold, against 6 after glucose.13 In 30 people with IBS, a low-FODMAP diet roughly halved symptom scores, and diarrhoea-predominant IBS was the only type whose stool frequency changed.14 The NHS lists too much fibre among the things that can make bowel leakage worse.12 Why fibre makes you bloated covers building up gradually.
How do you take psyllium for loose stools?
The pilot used 6 g a day, which its full paper calls similar to the two earlier trials; a trial dose, not a prescription.3 Stir it into at least 150 ml of cold water per sachet and drink it at once; the NHS adds that too little fluid can make constipation worse.5 Start low and, if stools turn hard, cut back and drink more: one in ten became constipated on psyllium in the 80-person trial.2 Take other medicines and supplements more than an hour before or after.15
Ask a doctor or pharmacist first if you have difficulty swallowing or throat problems, tummy pain while feeling or being sick, muscle weakness of the bowel, or unusual changes in your poo lasting longer than 2 weeks, including blood; under-6s take ispaghula only if a doctor prescribes it.16 How to take psyllium husk and what psyllium husk does go further.
What does NICE say about fibre and IBS with diarrhoea?
NICE's IBS guideline asks professionals to review fibre intake, "adjusting (usually reducing) it" while watching symptoms, to discourage insoluble fibre such as bran and, if more fibre is advised, to use "soluble fibre such as ispaghula powder" or oats.10 For diarrhoea, the NHS suggests cutting down on wholegrains, nuts and seeds; a GP can refer you to a dietitian.9 A low-FODMAP diet, NICE adds, should come from a professional with dietary expertise.10 Which fibre is best for IBS goes further.
Can prune juice or sorbitol cause diarrhoea?
Past your own threshold, yes. Sorbitol is a sugar alcohol found in sugar-free sweets and drinks, and in prunes and prune juice, where it may explain the juice's effect on constipation; is prune juice good for you has the figures. NICE says "people with diarrhoea should avoid sorbitol", and food with more than 10% added polyols must carry the warning "excessive consumption may produce laxative effects".10, 11 With diarrhoea, the NHS advises against fruit juice and fizzy drinks: "they can make diarrhoea worse".4
What we don't know
- No trial in this evidence tested psyllium for diarrhoea from a stomach bug or food poisoning.
- The three psyllium trials were small, 162 adults in all, and studied bowel leakage, not IBS with diarrhoea.
- We found no trial comparing psyllium doses for loose stools.
- We found no trial that timed fibre-related loose stools or found the amount of fermentable fibre that loosens them.
Frequently asked questions
Should I take psyllium husk if I have diarrhoea?
For a stomach bug, fluids come first; no trial here tested psyllium. For ongoing loose stools a GP has checked, psyllium cut leakage in three small trials; take it in water as above.1, 2, 3, 5
Does fibre make diarrhoea worse?
It can: fast-fermenting fibre in a large dose, sorbitol and, with IBS, wholegrains, nuts and seeds.7, 9 Gel-forming psyllium reduced leakage of loose stools in the trials.1, 2, 3
Can psyllium husk cause diarrhoea?
Diarrhoea is not among the side effects the trial abstracts report; in the 80-adult trial, 10% became constipated on psyllium.2
What should I eat when I have diarrhoea?
The NHS says to "eat when you feel able to", that avoiding fatty or spicy food may help, and to drink lots of fluids such as water or squash; it gives no fibre rule for a stomach bug.4
Do live cultures help diarrhoea?
That evidence, including diarrhoea during a course of antibiotics, is set out trial by trial in do probiotics work.
